The Implications of VAT on Healthcare Services

I've spent years helping businesses of all kinds understand their tax obligations. But whenever I talk to clients who are hospital owners, clinic managers, or health-tech founders in the UAE, the conversation begins with the same question — "Healthcare is essential… so how does VAT even apply to it?"

It's a fair question. VAT was introduced in the UAE in 2018 to diversify government revenue and create a stable economy less dependent on oil. But healthcare deals with people, not products, so the system treats certain healthcare services as standard-rated or exempt, while many are taxed at 0%.

Most core healthcare services in the UAE are zero-rated: VAT is charged at 0% on surgical procedures, diagnostic services, preventive care, medical consultations, dental treatments, and the like. Here you can still claim back your input VAT, which makes a real difference to your balance sheet.

Some healthcare services are exempt. This sounds similar to zero-rated, but the impact is different: no VAT is charged to the patient, but the provider cannot recover input VAT on its expenses. For healthcare operators, that distinction directly affects profitability and can increase operational costs.

Not every service offered by a hospital or clinic is medical treatment. Purely elective and aesthetic services fall under the standard 5% VAT rate — think cosmetic procedures, wellness services, sale of non-medical products, and hospital room upgrades beyond standard care.

This creates an interesting split in how clinics operate. If a facility does both reconstructive surgery (exempt) and cosmetic enhancement (taxable), its accounting system needs to be sophisticated enough to separate those revenue streams — you can't just lump it all together.
